With this document, we, the undersigned Nar-Anon members and fellowship representatives, acting on behalf of the above identified submitting entity, hereby submit the following input to be used in the above identified Work. The materials submitted herewith were prepared by us as members of the fellowship and working as volunteers within the fellowship structure.

It is the responsibility of the submitter(s) to have obtained all necessary clearances for inclusion in this piece. We recognize that our contributions will be considered as input to Nar-Anon World Services relating to the Work and will be used as appropriate. We agree that the Work is a "work made for hire" for Nar-Anon Family Groups, Inc., the world-wide service entity of the fellowship of Nar-Anon ("WSO"), or if the Work does not technically qualify as such, then, for valuable sufficient consideration, receipt of which is hereby acknowledged, we hereby assign to WSO all world-wide rights of copyright we may hold under any legal and/or equitable theories to the Work.
